This section of the Great Smoky Mountain Council's web site contains various
forms from the Council and BSA National offices. Feel free to download them,
print them, and use them elsewhere. Although we strive to maintain the accuracy
of these documents to the best of our ability, we do not guarantee any of the
information found here.
Everything here is available in one of two formats: Adobe Acrobat PDF and
Microsoft Word Formats. If you do not have Adobe PDF or Microsoft Word, it is

Medical Forms
Medical Forms are a requirement of the Boy
Scouts of America. All members should have at least a Class I medical
history on file (this is good for anything under 72 hours). However, if you plan
on doing anything over 72 hours, then look at Class II and Class III forms (both
require physical examinations).

for activities. Can't be used for Adventure Bases or Jamborees (use form
